Mission Statement:
The San Diego Miramar Sports Medicine Department specializes in injury and illness prevention, assessment, treatment, and rehabilitation services and athlete education for every student-athlete. The Athletic Trainer is committed to pursuing the changing needs of the student-athletes and the sports medicine community.
"A certified athletic trainer (ATC) is a highly educated and skilled professional specializing in the prevention, treatment and rehabilitation of injuries. In cooperation with physicians and other allied health personnel, the ATC functions as an integral member of the athletic healthcare team in secondary schools, colleges and universities, sports medicine clinics, professional sports programs, industrial settings and other healthcare environments." Steps to complete Preparticipation Exam forms.
2. Under Athlete/Parent (right side of page): click on “Join SportsWare”
3. Next screen: School ID - input MiramarJets, then click next.
4. Next screen: input first name, last name, email, select San Diego Miramar
That completes your request to create an account. Once your request is accepted by an ATC, you will get an email from SWOL to complete your account.
5. Once you log in – you will need to complete the Athlete info section, Medical History, and electronically sign all 5 PPE forms.
If you get a sports physical on your own, please use the form below. It must be completed and signed by an MD, DO, NP, OR PA.
